____ Credit card account numbers, balances, and monthly payment(s)

____ Monthly payment on real estate debt

____ Monthly income on investment property

____ Copy of divorce/separation papers (if applicable)

____ Two years of P&L (if self-employed)

____ Year-to-date P&L (if self-employed)

____ Two year's tax returns if self-employed or 5% or more of your
     income is derived from investments

____ W-2 and current pay stub(s)

____ Driver's license
